Founded in 1995, Element Care is a non-profit, healthcare organization with a simple mission: to help people live safely and comfortably in their homes and their communities for as long as they can, keeping their stays in hospitals and nursing facilities as minimal as possible. Element Care provides complete health care to eligible people through PACE (Program of All-Inclusive Care for the Elderly) throughout the Massachusetts North Shore, the Merrimack Valley and the northeast region of Middlesex County.
Element Care Block Rotation
The rotation is approximately 4 weeks. The programs provide interdisciplinary care and the fellow will experience how to manage the participants with the team to provide cost-effective care in the community. The team at the site will include a nurse practitioner, social workers, nurses, physical therapists, occupational therapists, personal care assistants, integrated mental health and visiting dental, podiatrists, and acupuncture.
There are up to 150 frail elders per site. Care is provided at the center, home, nursing home and hospital for these patients. The continuum of care across multiple provided by one team enables the fellow to understand the ability of the patients to live supported by the program in the community and to understand the smooth transition of care.
The fellow is expected to perform routine physicals, address acute issues and participate in participant care meetings.
